APPLICATION CHECKLIST

Indonesian Students must submit :

• Completed Application Form for Admission
• Bank Receipt (if you have mad School Fees Payment through Bank Transfer)
• Copy of both Parent's ID Cards (KTP)
• Copy of Family Card (KK)
• Copy of Parents Marriage Certificate
• Copy of Child's Birth Certificate
• Copy of Child's Report Book (certified by the school)
• 8 Coloured 3x4 photograph of Your Child
• Copy of Ijazah & SKHUN in Y6 (for enrollment in Year 7)
• Copy of Mutation Letter from Previous School
• Copy of Passport of Parents

Foreign Students must submit :

• Completed Application Form for Admission
• Bank Receipt (if you have mad School Fees Payment through Bank Transfer)
• Copy of child's and both parent's KITAS and Passports
• Copy of child's report book (certified by the school)
• Enrollment letter from the current school to confirm student's status
• 8 coloured 3x4 photograph of Your Child
